OUR COMMITMENT TO YOUR PRIVACY

Radiant Recharge LLC is committed to maintaining the privacy and security of your protected health information (PHI). PHI includes information that can be used to identify you and that we have created or received about your past, present, or future health or condition, the provision of health care to you, or payment for health care services.

OUR OBLIGATIONS UNDER HIPAA

Under the Health Insurance Portability and Accountability Act (HIPAA), we are required to:

  1. Maintain the privacy of your PHI.
  2. Provide you with this Notice of our legal duties and privacy practices with respect to your PHI.
  3. Follow the terms of this Notice currently in effect.

HOW WE MAY USE AND DISCLOSE YOUR PHI

We may use and disclose your PHI for the following purposes:

  1. Treatment: We may use your PHI to provide, coordinate, or manage your health care and related services. For example, we may disclose PHI to specialists as part of your treatment plan.
  2. Payment: We may use and disclose your PHI to bill and collect payment for the services we provide to you. For example, we may provide PHI to your health insurer to get paid for services.
  3. Health Care Operations: We may use and disclose your PHI for our health care operations, which include activities necessary to run our practice and ensure that our patients receive quality care. For example, we may use PHI to evaluate the quality of care you receive or to conduct training programs.
  4. Required by Law: We may use or disclose your PHI when required to do so by law.
  5. With Your Authorization: We will obtain your written authorization before using or disclosing your PHI for purposes other than those described in this Notice.

YOUR RIGHTS REGARDING YOUR PHI

You have the following rights regarding your PHI:

  1. Right to Inspect and Copy: You have the right to inspect and copy your PHI.
  2. Right to Amend: If you feel that the PHI we have about you is incorrect or incomplete, you may ask us to amend the information.
  3. Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures we have made of your PHI.
  4. Right to Request Restrictions: You have the right to request restrictions on certain uses and disclosures of your PHI.
  5. Right to Request Confidential Communications: You have the right to request that we communicate with you about your PHI in a certain way or at a certain location.
  6. Right to a Paper Copy of This Notice: You have the right to receive a paper copy of this Notice upon request.

CHANGES TO THIS NOTICE

We reserve the right to change the terms of this Notice at any time and to make the new Notice provisions effective for all PHI that we maintain. If we change our Notice, we will provide a copy of the revised Notice to you upon request.

COMPLAINTS
